When you request therapy or an assessment, you use our secure screening form and client portal, which are hosted by SimplePractice, or you contact us by phone or email. Information you submit there is handled under our Notice of Privacy Practices and SimplePractice’s own security safeguards. This website itself does not store form submissions or health records. Please don’t include personal health details in email.
Information collected automatically
